Job Description:
A company is looking for a Newborn/Pediatric Nurse Care Manager
to provide holistic care and support to members through their
healthcare journey.
Key Responsibilities:
Deliver coordinated, patient-centered virtual Care Management via
telephone and/or video
Generate impactful care plans with members and the
multidisciplinary care team
Assist members in navigating complex medical conditions and the
healthcare system
Required Qualifications:
Bachelor of Science in Nursing (BSN)
Active Compact RN license in good standing
2+ years of experience in a Newborn/Pediatric setting
Current CCM Certification
Willingness to work Monday-Friday, 9am-6pm PST
