Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical needs.
Key Responsibilities
Evaluate member needs and recommend care plans for optimal
outcomes
Collaborate with providers to address members' unmet needs and
coordinate care
Monitor member progress and provide education on healthcare options
and benefits
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse State Licensure and/or Compact State
Licensure required
